Clinical feasibility of targeted cardiac resynchronization lead delivery using a 3D MRI cardiac model
نویسندگان
چکیده
Background Cardiac resynchronization therapy (CRT) aims to reduce dyssynchronous contraction through simultaneous pacing of the right ventricular (RV) septum and left ventricular (LV) lateral wall. Up to 40% of patients do not respond, largely attributed to lack of dysynchrony and/ or transmural scar at pacing sites. In this pilot study we tested the feasibility of guiding LV and RV leads to “optimal” segmental targets using a MRI-based 3D surface rendered cardiac model.

Model-based navigation of left and right ventricular leads to optimal targets for cardiac resynchronization therapy: a single-center feasibility study.
BACKGROUND Left ventricular (LV) and right ventricular pacing site characteristics have been shown to influence response to cardiac resynchronization therapy (CRT). This study aimed to determine the clinical feasibility of image-guided lead delivery using a 3-dimensional navigational model displaying both LV and right ventricular (RV) pacing targets. Serial echocardiographic measures of clinica...
متن کاملWhole heart magnetization-prepared steady-state free precession coronary vein MRI.
PURPOSE To compare two coronary vein imaging techniques using whole-heart balanced steady-state free precession (SSFP) and a targeted double-oblique spoiled gradient-echo (GRE) sequences in combination with magnetization transfer (MT) preparation sequence for tissue contrast improvement. MATERIALS AND METHODS Nine healthy subjects were imaged with the proposed technique. The results are compa...
متن کاملALternate Site Cardiac ResYNChronization (ALSYNC): a prospective and multicentre study of left ventricular endocardial pacing for cardiac resynchronization therapy.
AIMS The ALternate Site Cardiac ResYNChronization (ALSYNC) study evaluated the feasibility and safety of left ventricular endocardial pacing (LVEP) using a market-released pacing lead implanted via a single pectoral access by a novel atrial transseptal lead delivery system. METHODS AND RESULTS ALSYNC was a prospective clinical investigation with a minimum of 12-month follow-up in 18 centres o...
متن کاملCombined preoperative information using a bullseye plot from speckle tracking echocardiography, cardiac CT scan, and MRI scan: targeted left ventricular lead implantation in patients receiving cardiac resynchronization therapy.
AIMS To evaluate the feasibility and incremental value of using an integrated bullseye model for presenting data from cardiac computed tomography (CT) and magnetic resonance imaging (MRI) in combination with echocardiography evaluation of segmental mechanical delay for guiding optimal left ventricular lead placement in cardiac resynchronization therapy (CRT). METHODS AND RESULTS Thirty-nine p...
متن کاملMapping cardiac fiber orientations from high-resolution DTI to high-frequency 3D ultrasound
The orientation of cardiac fibers affects the anatomical, mechanical, and electrophysiological properties of the heart. Although echocardiography is the most common imaging modality in clinical cardiac examination, it can only provide the cardiac geometry or motion information without cardiac fiber orientations. If the patient's cardiac fiber orientations can be mapped to his/her echocardiograp...
